A Clinical Use of Serum ¥â-hCG, Progesterone and Alpha-Fetoprotein Levels for Differential Diagnosis of Early Abnormal Pregnancy

Abstract
A prospective clinical study was done to evaluate usefulness of serum ¥â-hCG, progesterone(p4) and alpha-fetoprotein(a-FP) determinations in normal intrauterine pregnancy, abnormal intrauterine pregnancy(including blighted ovum, incomplete
abortion
and
missed abortion), and ectopic pregnancy. Serum samples and medical records were prospectively analyzed with 40 cases of normal Intrauterine pregnancy, 60 cases of abnormal Intrauterine pregnancy, and 60 cases of ectopic pregnancy.
P4, ¥â-hCG and a-FP were highest in normal intrauterine pregnancy. The results showed that there was significant difference in serum ¥â-hCG, P4 and a-FP. Serum ¥â-hCG, P4 and a-FP(¡ÃGestational period 10 wks) were significantly lower in abnormal
intrauterine pregnancy and ectopic pregnancy than those in normal intrauterine pregnancy, whereas those were not significantly different in abnormal intrauterine pregnancy and ectopic pregnancy.
A combination of biochemical markers including ¥â-hCG, P4 and a-FP were applied usefully in early detection between one group, normal intrauterine pregnancy and other group (abnormal intrauterine pregnancy and ectopic pregnancy), We think that
additional diagnostic aids such as sonogram, diagnostic D&C and /or laparoscopy should be supplemented in order to differentiate between abnormal Intrauterine pregnancy and ectopic pregnancy.
